Scheduling an Appointment

To schedule an appointment, please call us at (847) 884-8884. Please have your insurance information available and a referral for treatment (if your insurance policy requires one). If you have previous records from other treatments, these records can be forwarded to our office prior to your appointment.

We encourage both partners to attend the initial appointment. We will thoroughly review your medical history and any previous treatments. Your physician may perform a vaginal ultrasound to evaluate your ovaries and uterine cavity. Once your consultation is completed with the physician and a plan for treatment is decided upon, you will then meet with a primary nurse to review all the work-up testing needed and to review your treatment plan in detail. This is an opportune time to have your immediate questions answered. We are committed to answering any questions you may have regarding your care and treatment.
